About the 267 Forms on CD...
This product is an accumulation of forms from clients, professionals, online resources, and the like. Every effort has been made to assure that the contents are current and usable by most practices.
A majority of the forms are in Microsoft Word format and can be cut and paste into a new document for editing/customization. You may also use the forms as-is. In some instances, it was not possible to include a form in the main Word document. When this is the case, the page will refer to the folder where the actual form can be used. The Superbill forms are in Microsoft Excel format.
Note: The Superbills do not contain ICD-10 codes as there is no way to condense these into a usable format.
Formatting: Please note that shifting may occur due to variances in your computer, font type, font size, margins, page size, etc. The end-user should be familiar with the functions of Word and the capabilities of manipulating text for printing purposes.
